I am currently pursuing "borrower defense to repayment" the DOE does not provide a standard form, but this is a URL to a PDF form that can be edited :
https://lnkd.in/bqW98eT
The form is sent to via email to FSAOperations@ed.gov or you can mail directly to:
U.S. Department of Education
PO Box 194407
San Francisco CA 94119
For more information read this material provided by the DOE: https://lnkd.in/bU8Zkge
Remember this is not a formal Court document you have to be honest, but if the only evidence you have is your story or memories, that is enough!! The more evidence the Department of Education receives about any particular school the more evidence they can put forth to show an atmosphere of systemic misrepresentation.
Thank you for your time.
Any many of you reading this forum will want to bookmark this page for 5 years from now when all the promises half-made by your respective deans of admission don't come true.

In case anyone's concerned, the above are just links to the DOE forms you can fill out if you're seeking to discharge your student loans on the grounds that your school defrauded you or violated state law. (Some of you may remember jdcumlaude from various Campbell Law threads.)

Yeah sorry about the strange URL. I took this directly from a Linkedin post and that website automatically does their own version of tinyurl due to their character limit on update postsA.
