Actually, ’87 was not an anomaly, what happened was USNWR modified its rating criteria, thus the changes.
USNWR is changing its criteria for ’13,

I'm aware. It's still an anomaly: an anomaly is just something different; it doesn't itself imply anything beyond that.

So, do we have any idea who coined the phrase? I think we have a justification for the title, but how did T-14 become a commonly used term?

it was coined by Sir Thomas the 14th, the first known American to graduate from a "top ranked" law school.

The older people I've talked to refer to the good schools as "the top 15 or 20."

Negative. It is a term that evolved
because of USNews. The top 14 have essentially been the same since USNews started ranking law schools. (Texas and UCLA have snuck into #14.) But more importantly, the T14 have all ascended to the top 10 at one time or another. (And this is why Vandy and UCLA have a big leap ahead if they are to strive to enter the halls of T15.)
Note however, USNews does not use the term itself. It has just become common knowledge.
