If you are not comfortable letting them handle the form. You can simply have them mail a standard letter of good standing to Columbia as a substitute of the dean's appraisal form. Hope this helps. I wouldn't be too worried, historically this seems to be very early, and I am sure there are PLENTY of more admissions coming out.
