Writing an Addendum for "any formal or informal" discipline

A couple of schools that I'm applying to ask the question:

"Have you ever been subjected to any formal or informal undergraduate or graduate school disciplinary action for behavior (whether committed on or off campus)?"

in their character and fitness portion of the application.

I went to a private Christian school, and during my sophomore year, I had a male friend over in my on-campus apartment a little after curfew for a type of opposite gender visiting hours. My RA saw him leaving my apartment and had me go speak with the RD about the rules for the opposite gender visiting hours. Should I answer the question above with a 'yes' and write an addendum explaining this?

Also, I received a parking ticket once for parking in a visitor parking space at my school. Should I also write an addendum for this, because at least one school's character and fitness portion of their application asked if the applicant has received any parking tickets/violations.

Yes, you should post an addendum because that does fall under the parameters of the question. No, they won't care, because that situation is so absurd it will prompt laughs way before it prompts concern.

Then again, if there was no paper or record, you may not need to bother reporting that you were (gasp) visited by a guy at nighttime.

Disclose the parking ticket on the apps that specifically ask for them, most don't care.
