Unless your reason for those bad grades is a good one (think death in the family, debilitating medical issues, etc.), don't send the GPA addendum.
Send the DS. Here's the email I sent to update my application with something new. Feel free to reformat to your needs.

Good morning!
I'm so sorry to bother you with this, I know how busy your office must be this time of year. If it is possible, I would like to submit an updated version of my (1) Personal Statement, (2) Academic Probation Explanation, and (3) Résumé to my submitted application. According to my Status Checker the application has yet to be reviewed for completion, so I'm hoping it won't be too much trouble! I have attached the three documents to this e-mail in .docx format.
