if the professor said they'll write the letter then they'll write it. it might be good to touch base with them just to stay in contact, but i don't think you need to verify that they're still okay with writing it for you

Yeah, I hear that. Its been like 6 months though since I met with her/she said she would write it. I have two other recommenders, who both wrote their letters within a month. IDK, now I'm concerned with whether or not the letter will be positive in my app. I know that LORs are marginal, if not negligible, in the adcoms eyes in the whole process, but I still wouldn't want to submit a letter if it wasn't a good one.
Then just submit the 2 good ones. I don't recall any school I applied to requiring more than 2 letters, though they would accept more. No sense in adding a 3rd letter if you're not sure if it will even help you. As long as your PS/LORs don't have some kind of fuck up you'll get in wherever your numbers will take you.

I think it's probably fine to send an email to follow up with your professor, but this one is far too long. Also, you seem very insecure about it. I would assume that the professor has either forgotten about it or has been very busy and put it on the back burner. However, I would also assume (probably correctly) that if the professor felt uncomfortable about writing the LOR, she would not have agreed to do it in the first place.

Having said that, I would recommend the following (fwiw):

1. Remind her of your request, including the date of the request. For example, "Back in May we met to discuss my request for a letter of recommendation, and I provided the necessary forms to you" etc..

2. Inform her of your upcoming deadline. For example, "I am preparing to send out applications soon, and I want to make sure that you will be submitting your letter by xx/xx/xxxx."

3. Give her a chance to back out or give you another date. "If you are unable to have the letter completed by xx/xx/xxxx, please let me know when I should expect it to be submitted. "

4. Close it. For example, "Thanks again for agreeing to write a letter of recommendation for me."

I don't think you need to mention what you've been doing during the summer.
