I am a military service member (enlisted) seeking to go to law school. I will obtain an LoR from my current supervisor, who is also a Soldier.

Can anyone tell me where I can find a solid sample LoR to give to my boss as reference material on what it should look like?

Also, he has worked with me for about a year full-time so knows me very well, but what material should I provide him with when asking for a LoR? (e.g. why I want to go to law school perhaps? A writing sample?!)

Ivey's guide for law school admission has a few good samples of good LOR's and bad LOR's.

I went all out in terms of what to give my recommenders... I don't know if it helped or not. I gave them all my resume, my first draft of my PS (which is nothing like what my final PS will be), and a copy of a few blog posts from Yale's admission blog about what they look for in recommendations (none of my recommenders had written a law school recommendation before). I'm afraid the last part came off as a bit pretentious, but none of them seemed to mind.

eh. I think you're trying to hard. I gave my professors my resume, the LSAC form, and a stamp....I think I made out pretty well.
