I think the consensus is that even in the best of circumstances, GPA addendums really don't accomplish much. Writing one up for one bad grade in one semester will probably be overkill. If your other grades that semester were OK, I think that would be even more reason to not write an addendum.

I wrote a GPA addendum to explain a class I failed when I was 14 years old, and mainly because of how early in my academic career I took the class. It didn't seem to do much for my cycle. If it had occurred any later in my life I don't know that I'd have bothered.
