One class Addendum

I am writing an addendum for one class. I was supplementing my courses at my University by taking classes at a community college. I was apparently signed up for a class (Spanish 3) which I never attended and so I received an F. I took the class at my regular university and didn't even realize I had been signed up for the class at the community college until I went about collecting transcripts for LSAC. Any recommendations on how to address this is an addendum, without sounding like a complete idiot for the mistake. Any help is greatly appreciated!

I'm not entirely certain how you sign up and pay for a class that you never "signed up for". But assuming you did, call the community college to obtain records of your monetary transaction to figure out what happened. If it appears that you never attended the class even one day request they remove the record and thereby be exempt from having to report it to LSAC at all.
If you did accidentally sign up, pay for the class, and never request a withdrawal then explain the situation as is, I suppose. Reference taking the class at the university. Hopefully you scored a B or better for persuasive purposes.
