167/3.47 GPA; Minnesota, Notre Dame, Indiana--medical issue

Hi, the top three schools I'm applying to are Minnesota, ND, and UofI. I have a decent LSAT of 167.
My GPA (at Ohio State) isn't that great (3.47) because of a poor senior year. I am epileptic and during my senior year I was having nocturnal grand mal seizures frequently. These often left me unable to get out of bed in the morning, and when I could get out I was in a hangover-like state that often follows the day of a seizure. This impacted my attendance and my performance on midterms and finals. For the past three years, I have been seizure free. I am including an addendum to address this issue.
I am 25 and have been out of school for three years. One year I spent doing Americorps on an Indian reservation; I spent six months as a corrections officer at a women's prison, and I spent the last year as a director of a non profit I founded. The mission of the non-profit is to achieve repeal of the death penalty in South Dakota through legislative means.
Anybody have any thoughts on my admission chances for Minnesota, Notre Dame, or Indiana?
