You will get into a top 14 no problem. I'd blanket the lower half, and frankly with the URM status, I'd blanket the entire shebang. You have a chance with Harvard, Stanford, Chicago, NYU and you are pretty much in everywhere else. That 158 isn't a big deal if you just state in your addendum that you took the LSAT very cold and with no prep the first time, but that when you were able to approach it seriously, you scored that 171. Just make them understand why the 171 is more indicative without making excuses.
Also I wouldn't do an addendum for the GPA. At best it does nothing (everyone has a better degree gpa it seems) and at worst it comes off as an excuse.
You are selling your numbers short. URM is a golden ticket- use it man/woman.

I wouldn't do an addendum for the LSAT score. People don't usually get 171s by chance. The 171 speaks for itself.
