Personally, I consider any school where you are under the LSAT median as a reach, even if your GPA is at the median or slightly higher. Now, if your GPA is over the 75th percentile, you have a better shot.

I think the terms really apply to you depending on your split between GPA and LSAT. For instance, I have a relatively low but not disqualifying GPA and a good LSAT, so 11 of the 14 schools I applied to are considered "targets". Just depends on who thinks its good enough and who doesn't.
Edit: To clarify, I agree with earlier poster about "anywhere you are under LSAT median" as a reach. Safety would be hard to define, but in my case it was any schools where my GPA ~ median.
