Well some schools do rank the waitlist (i.e. Berkeley). As per your question regarding medians, schools can choose any applicant that they want. If their GPA median is low they may take a reverse splitter. If the LSAT is low they may take a splitter. Schools also look at other factors such as geographical location. If it is late in the cycle they may be more enticed to accept a student that is closer to the school and would be able to come on a last minute basis. Ultimately there are a lot of moving parts so it is hard to predict. But, schools will always keep medians in mind since they matter for US News Rankings.

Most people I know who got into t14s off the waitlist were either splitters or reverse-splitters.

I would be very pessimistic about someone's waitlist chances if they were non-urm and below both medians.
