Disciplinary Action

"Have you ever been subject to disciplinary action for any scholastic, judicial/code violation, or any other reason by any college, university, graduate or professional school, trade or professioanl organization?"
Does disciplinary action mean academic probation? I was placed on this my freshman year.

Yes, scholastic is synonymous with academic in this case.

And even if it didn't, the words
"...or any other reason..." foreclose the omission of any discipline durung UG.

Also, for the addendum that is required by answering yes... I am planning on putting all this into a GPA Addendum. Is that correct?
Do I have to specify what semesters I was placed on academic probation in the addendum? How would I find this out?(it's not on my transcript).

Yes, you need to detail everything. You need to tell them what the sanction was specifically, how long the probation lasted, and how it was not severe enough to result in a violation on your transcript. I would not conflate this addendum with your GPA one.

One too many LORs, axe the cover letter. Other than that, you're probably good to go.

Maybe not, but some schools won't even accept more than 2. Letters of recommendation aren't really that useful unless you get a bad one.

I applied to ~25 schools, and every single one of them required the disclosure of academic probation. Sometimes it was under a different banner than 'disciplinary action,' but they all had it. As such, I disclosed it to every single school that required it.
In the end, having been placed on academic probation because of grades is not usually going to hurt you in and of itself. (I managed to sneak into a T10 school despite of—or mayhap because of—the circumstances surrounding the probation.) The GPA will be the killer, though.
