So my September 2016 tests were lost in the mail and I had to retake with a new, undisclosed test this past weekend. Should I include an addendum about this with my applications or will LSAC notify schools that my September score is a retake?

Yes, I wound addend for 2 reasons:

1. Wouldn't trust an organization that lost the most important test you've taken yet to do anything on your behalf.
2. If your score is affected by the retake you should mention that.

I guess it depends on what your LSAC account shows and if it is considered a retake (counting for your test limit), which I guess it "technically" is, but not by TLS definition.

So sorry you had to go through hell twice by no fault of your own.

Just that since I had to take a totally new test (with harder questions in my opinion than the original September test) with only two weeks' notice, stressed that they might get lost again, etc may have affected my score. Since it's still being reported as my "September 2016" score I was wondering if schools would be aware I had different testing conditions or if it was worth even mentioning at all.

If LSAC didn't tell you they were sending a letter noting test center disturbances or whatever they are calling this, then the schools won't know unless you tell them.

Though this really, really sucks and I feel for you, you really should retake if you underperformed. It's likely the schools won't care and will judge you by the score you have. In their mind, who's to say you actually would have done better?
