I scored really well on my meds. My advice would be to pick one state, either medicated or not, and do all of your LSAT work in that state. I never studied, took a PT, or did anything LSAT related off my meds. I completely agree with your concerns about studying in one state and testing in another. Do everything you can to accuratly simulate test day conditions.

I am prescribed Vyvanse (a stimulant, milder than Adderall but a stimulant nevertheless) and have found that it actually seems to be a hinderance during my PTs...particularly the RC sections. I cannot seem to rapidly absorb and synthesize the information in the text very well...which I suppose is likely side effect of the increased cognitive arousal/anxiety that comes with taking a stimulant. Sometimes I'll end up having to reread the same paragraph 2/+ times just to grasp what I'm reading; this is not really an issue without the Vyvanse, however.
It's not really a problem while studying...just during the actual tests. I know the simple solution would seem to be "well, then just don't take it during the test." But my concern is that since I have done most of my prep while on the medication, I may ultimately perform differently without it.
I read somewhere that it is best to take an exam in the same mental state you were in while studying for it.
Has anyone else experienced this?
Try doing one preptest without your meds. See how it goes. How did you score? Was a positive experience or were you completely thrown off? You can weigh the pros and cons and see if you want to change it up or continue prepping as you've been doing.
From my experience with students who've had this issue, you're late enough in the game where you'll probably do better with your meds, but it's impossible to predict that unless you experiment.

I noticed that as well. Sometimes I'll get bogged down in the minutia of a passage or LR question and be really into something that doesn't matter. Other times it's the opposite and I'll sort of feel like a zombie and not be distracted, but not be alert and focused on the actual passage.
Try it off your meds for a PT, but I think the best way is to just really push yourself and use the meds. Not using them can really hurt if you get really distracted on the LR or whatever.

I have prepped for and taken the LSAT twice, both times in 2013. I have adhd and dyslexia, but was not using medication at that time. I did pretty hardcore prep (PTs 1-66 timed twice with an extra RC supplemented, PS bibles, Manhattan books). For second prep, my peak PT was 176. My average was 173. I used to down a bottle of Coke Zero and a bag of frosted shredded mini wheats mixed with pretzel m&m's after third section, which was a combo I sweared by.
Anyway, not sure if any of this will help you, but wanted to share it was possible for me to do fairly well with no meds, working FT. I am starting my (hopefully) final prep this week, but going to do it medicated. I think adderall will likely prove a more effective stimulant than my old sugar/caffeine combo but I totally know what you mean about getting hung up on unimportant details. It will be interesting to see how it goes. Good luck in your prep
