How do you deal with "stupid mistakes"

In the last couple of prep tests I find my mistakes to be:

didnt read the other answer choice, misread a word, glanced over the last sentence in a stimulus, misread a word, or silly mistakes like this, do you think I am simply just lacking focus?

That was the answer I came up with as well! I notice that I try to treat each Q as same difficulty, taking same amount of time on each Q. I will def adjust more time/slower for difficult Qs, as more conscious effort.
