I feel like I'm hitting a wall in my prep work with LR. I've gotten my speed and overall confidence up, and I'm waffling back and forth on less questions. However, in each section, there will invariably be 2-4, lately usually 2 questions that I look at and can't see a single good answer choice. I have yet to get an overall perfect score in both sections combined, and my mistakes don't seem to take on any predictable pattern. I average anywhere from -3 overall to -4, which doesn't put me in a good place for the other two sections. These mistakes usually center around me misunderstanding a few words, or misinterpreting a phrase, and I don't know how to improve on that.

Any advice from people who have gotten over this hump?

Wish I could help but that's pretty much where I'm at as well. Good days I can go -1 (I've yet to ever get -0... always seem to finish early and then go back and change a right answer to the wrong one after second guessing myself) and on bad days I'll go -4. Hoping it just takes more practice and familiarity.
