I cancelled my June exam on-site, and I was wondering if I should also re-confirm my cancellation my mailing the form. Has anyone heard of any cases where the cancellation on-site didn't work reliably?

Yeah, it has failed for people that didn't fill it out properly and fill in both bubbles.

If you want to make sure, just fax or mail in the form too, filled out properly and signed.

You should receive an email from LSAC confirming that you cancelled your score some time soon.

Yes, I think if you are worried enough to make a post then you should send in the cancel form just in case via fax/mail. Better safe than sorry.
