They say the score cancellation form would be available immediately after taking the test on your account. Yet, it's not available on my account. My account actually still says my ticket for the test I took yesterday is ready to print :/

I've been told this is the form you can send in to cancel. It's the one I'm faxing in before noon tomorrow:
