How Common is Billing Fraud in Law?

I've heard that billing fraud is not uncommon in law and curious if people know how common it is and at what levels of law does it occur most?

Fraud might not be the right word, but there's definitely padding going on. Thing is, it doesn't really matter. If you, as an associate, spend 3 hours on something that should ordinarily take 1, don't think for a second that the client will be paying for 3 hours of your time.
