What exactly is law review?

I just know it's usually the top 10% of a class that gets it, and that it's not just a title. What does it entail, though? Any new responsibilities? Is it worth gutting myself for that top rank?

Generally no. At most schools each journal has a right of first refusal. This gives whatever journal you are on the right to publish your journal article and if they deny it then you can send it to other journals.
