Does every school have some sort of form that I can fill out to let them know I want to withdraw from the school? I would like to withdraw from 13 schools so I can give up my spot to people on their waitlist, but am not sure if there are necessary steps I need to take.

Print out your application. Walk it through middle earth. Avoid cave trolls, orcs, wizards, armies (Uruk-hai, human or otherwise) etc. Throw it deep into the fires of Mount Doom.

I searched TLS for an email template and sent that.

Don't pay your deposit, that's a pretty good indicator you won't be attending.

I had attached a detonator to my application so withdrawing was done at the push of a button

This is what I know:

1. Email the schools' admissions offices and advise them that you are withdrawing your application.

2. Then Call the School for the follow up if you really want to be sure that they receive your withdrawal Email

3. I am not sure about this but some will take your email and some will ask that you write a letter and mail it in.
