New York Character and Fitness (supervisor at new employer)

Hey all, going through C&F (NY). If our former supervisor (legal) moved to a new firm, do we provide contact details of them or HR from the old firm? I am assuming they will call/email (or do they just email and ask whether you haven't screwed up etc).

It’s been a while, but when I did CF in New York, they mailed a questionnaire to my supervisors/references
