What exactly does this mean for LSAC grade summarization

I was typing in LSAC grade summarization and found this other site:

My question is -- Can someone explain to me, what exactly it means on the GRADE CONVERSION TABLE on the attached website, what FOUR PASSING GRADES (highest 4.0) means?
Are they talking about grade symbols that have 4 passing grades? Or are they meaning if you retook a class three or four times that you passed? This just made no sense to me, because I was under the impression that every time you retook something all grades got factored in?
