NY Character & Fitness Supplemental Affidavit

So I was an idiot and forgot to disclose something that was in my law school application. Honest mistake, but they have asked me to explain the failure to disclose in an affidavit. Couple questions:

1) Anyone know what format/template to use for this?

2) Suggestions for content/phrasing? I know candor is most important and I need to tell the truth, but anything beyond that would be helpful.

3) I called the division where I'm being admitted and they said it was likely not a big deal, but how concerned do I need to be?

Cop a form affidavit from Lexis/West. Write your story, sign and get notarized.

Presumably, something like:

Affidavit of Oopsie daisy

Oopsie daisy being duly sworn, states:

My bad.

I declare under penalty of perjury that the foregoing is true and correct.
Executed this 1st day of April, 2014 at Top Law Schools.
