How do people make lower than a B+ on exams?

So at my T6, the median on any given exam is a B. With roughly the top 20% of exams getting A/A- and the bottom 20% getting B-/C.
I really don't know how its possible to make lower than median on law school exams. I've never made lower than a B+ on an exam in my law school career. And I've only made 2 B+'s, both of which were on exams where after reading the question I was so confused I didn't know where to start. I literally spent most of the test just reading the question hoping to see if there was something I was missing. I barely wrote anything in my response. I know alot of people when they panic just start writing everything they know down. I really was so confused during these two tests that I didn't even know where to begin.
I left both exams totally devastated expecting to make B-'s at best and most likely getting a C on the exam. Yet somehow I made a B+ on both exams placing me in the top 50-55% of the class. So I got to thinking, if my answer as bad as it was didn't make the bottom 40%, what the hell did those folks write to get them such low grades?
Can someone clue me in on how people make such low grades on exams?

Unless median means something entirely different as it relates to law school exams, I don't know how it's possible for someone who managed to get into a T6 to not know what median means.

I've always wondered how people can go through life without having any friends. OP can you clue me on on how this happens, I have a feeling you'll be able to explain it.

I know that someone has to be at the bottom of the grading range. My question was about how bad a test must be to get lower than a B+.
I seriously am in shock right now because I got a test back that I fully expected to make a B- or C on. I didn't know anything on the test, barely read for the exam, only studied the night before and still wound up getting a freaking B+.
I seriously question how any of my classmates could have written a worse exam? Maybe I'm getting lucky and profs really don't read what we write, but I'm seriously in shock over how I made a B+ on an exam that I was sure would be terrible.

I felt devastated after walking out of my crim final last semester... then it turned out I got an A+.
How the fuck can someone not get an A+? It's not as if they just line up the grades and arbitrarily cut off the bottom 20 percent regardless of how close they were to the median. Jesus Christ, I must be surrounded by dumbasses.
