Bar Application- Can't remember employer from 10 years ago!

I briefly worked for a commission only paying job in 2002 and once I realized it was a pyramid scheme I quit. I can't remember the name of the company and I honestly doubt they are in business anymore. But, I've got to put this on my bar application. I have no clue what to do. Anyone have any suggestions?

Did you ever even get paid? If not, I doubt you need to consider this a job.

Did you receive a 1099 from them? If yes, did you file it with your return that tax season? You may be able to find out the name of the employer by contacting the IRS. All 1099s have the employer's information on it such as name, addresss and EIN (employer identification number). I would definitely contact the IRS to see if you can get a copy of that return.

Edit: The Social Security Administration will provide you with your detailed employment history but they may charge you and it could take up to four months. I know time is of the essence so I would just go down to my local IRS office first.
