Any tips as to how to structure a conlaw outline for optimal usefulness during the exam (and edification while writing it)? There aren't clear elements a lot of the time, and going case by case would miss the connections between them. Thanks for any suggestions!

Idk if there's one correct answer here. IMO it should be structured according to how your professor specified. Usually for con law this will be frameworks with factors.
eg: commerce clause - Lopez 3 categories (channels, instrumentalities, substantial affect). then what would constitute a substantial affect according to your professor (my professor gave us certain factors to consider under that category).
eg: spending power - SD v. Dole framework
eg: executive action - Jackson concurrence framework (according to my professor)
I think you get the idea. That's my opinion. Basically tailor it to your professor. HTH
