Like, a student from a school with a B + median is transferring to a school with a 3.0 median?

Why not? What happens if you have the space in your suitcase for it? Do they not allow it through airport security? What happens if you drive it in?

Most schools only give you the unofficial transcript to prevent just such a thing. The official ones have a little strip embedded in the paper that you can see if you hold it up to the light. If you have one of those then yeah, you'll be able to bring your grades (by car, not airport due to security).

how would OCI work then? Would employers just consider my first year grades from my old school?

Yes. This is why transferring may or may not be a great idea for people who are excelling at a given school - if the employers at the new school's OCI typically didn't hire people out of the original school, they may not be particularly interested in hiring you just because you transferred to a school where they do hire. Probably not so much an issue for you if you're looking to transfer down, though.
