Does CLS need UG Dean's Cert before admission?

...or does it work much like their regular 1L admission where they only need it once you are admitted in order to enroll?

I received an email from CLS today requesting a Dean's Certificate in order to deem my file "complete". So, yes you need it before hand.

You need it for them to review your application.

Can anyone explain the logistics of how they submitted this form from their undergrad? Did you fill out the information on your end, fax it to them and then have them mail it? My undergrad is a plane ride away so I can't go in-person.

You are correct... You fill in the designated portions of the form (you can even type it out, so no need to print it out and fax it if that's an issue for you), then email in the form to your undergraduate institution and they will complete the rest of the form and send it out for you.

You used an e-signature?
Personally I wrestled with my UG to find someone to handle this for me and got her email. Then I printed, filled out, signed, scanned, and emailed the form to her. She then printed it and filled the rest in and is mailing it.
