I was in a similar spot last year. Watching the specific schools' application threads, there isn't too much rhyme or reason for t14 schools acceptance of top T2 students. It's such a black box process. Some have larger transfer class sizes, which may actually give you a better shot at a higher ranked school than you would think... Some admissions office may just take an interest in your app or something.
I think top 5% gives you outside chance anywhere except maybe YS. H would still be very much long shot, t6 would be long shot, rest of t14 with small transfer sizes would be outside chance. Probably in at t14 with large transfer class schools.
Probably in at any other schools worth transferring into unless they have small transfer class.
My advice would be to apply anywhere in t14 you'd be willing to attend. The more applications you have out there, the better your chances. I regret skipping out on some of the applications.

I was in your shoes a few years ago, and wanted the same markets.
Your best bets to get in are GULC, Michigan and Northwestern. You can also probably get into Cornell, but they don't let transfers do OCI, so it's not smart to go there.
Penn/NYU/CLS are unlikely, but worth the application. You probably need to be in the top 1-3% to snag these, although there is always a chance. H is worth the application if you can get up to top 1%, but it's really hard for T2's to crack the very very top tier.
If you're from VA, apply to UVA; if you're from CA, apply to Berkeley; both of those are strong shots if you're in state. Otherwise, those schools are unlikely.

In your exact same shoes two years ago. Kill it 2nd semester, which is no guarantee and you have a good shot at a lot of places. I transferred from a mid-western school in the 60s and was ranked tope 7% and still and transferred to CLS (I was shocked I got in). I also got in at Penn and Michigan, but dinged at NYU. Play up local connections, that's what I did. But who knows, none of transferring makes any sense. Just do well.
