How do I identify all cases I've been part of for conflicts check?

Just got an offer and I'm filling out conflicts check form. I'm trying to remember a complete list of cases that I've been on, but I'm scared I'm going to leave out a case I forgot about. Is it customary for me to call up my previous firms and request a list of all cases I've been on, or how does this typically work? Obviously the first conflicts check I've ever done lol. Thanks in advance for all help.

eta: the language in the form is super broad, so even if I didn't enter an appearance, I need to identify the party I worked with.

I contacted my old firm (the alumni director or equivalent, or really any one) and asked them for what I needed. They got back to me within a week with all the information.

This part of practicing law sucks. I didn't even recall a third of the list they sent me. Specific business names of clients and adverse parties? Eyes glaze over at that. I just want to solve legal problems.
