What to Say When Giving Notice?

I am going to be giving notice to my current firm that I am leaving for another firm. I am going to call a senior partner to let them know. Any tips on how to say it or phrase it? Sounds like it should be easy but maybe I'm just nervous.

(Before responding, it would be helpful to know why you are leaving.)
Be polite & direct.
Give two weeks notice--or whatever is appropriate under the circumstances & expected by your firm.
Thank the partner for the opportunity to work for the firm.
Be prepared to answer: "Why are you leaving?" Of course, your answer depends upon the specific situation.
Do not seek revenge or display anger against anyone during this encounter.
If you are leaving for an opportunity to handle more substantive matters or to switch practice areas, then say so without assigning blame.
It may be okay to say that you need a change of environment in order to reenergize yourself.
If someone made you uncomfortable during your employment at this firm, then think about whether or not this should be addressed at all, as a personality clash, or with specific details.
Is there a specific reason why you are nervous about relaying notice about your upcoming departure ? If yes, then determine whether or not this is a matter that you want to share with HR.

OP here. Thanks for the advice! I'm nervous just because they've invested a lot in me and the conversation has been to keep me busy going forward so its just a natural feeling of disappointing someone.

Would also appreciate additional feedback on this. Thinking about potentially leaving the firm for another firm in the same city for the same practice (have a few offers in my hand but have not definitively made up my mind yet) for better career prospect. They have also heavily invested in me and the relationships with the partners are generally very good, but curious if there is a delicate, more professional way to convey the message. Maybe I am just worrying about something I shouldn't worry about in the first place, as I fully understand I will be easily replaceable. FWIW, mid-level corp assocaite.
