So I just accepted an offer for a 2L SA, on my resume I have a job listed as 2 months. I was still technically employed those two months, but I got into a car accident after a month and was sitting out for the second month.
Second, I was employed by a service that requires you to pay your own taxes (not automatically deducted from paycheck). I have never been contacted by anyone saying I owe tax money, I only made about 1400$ while employed there.

Will either of these things be grounds to revoke my offer?

No. But talk to an accountant to figure out if you owe taxes on the $1,400. As far as I know "never been contacted by anyone" is not a defense to not paying taxes

No. Taxes on the $1400 will likely just be auto-adjusted by IRS when they process your personal income tax return. That's something IRS processes automatically if the other guy filed a 1099. At least that's my experience with random 1099s that I didn't even know I had.
