What are the signs you’re going to be no offered at a medium sized firm? Are there any? Are good reviews and steady work promising ?

What's the historical offer rate at your firm? If high, then good reviews and steady work would be very promising signs.

I believe it is typically 6/7 or 7/7 get offers but it’s hard to get answers on the offer process from anyone

Going off of this, is it a bad sign if you don’t receive an offer on your last day ?

At my firm that no offered 1/7 summers almost every year, it meant that someone wasn't getting an offer. But every year everyone, usually including the person being no offered, was aware of the situation.

If the historical offer rate is that high, then your good reviews and steady flow of work strongly suggest you won't be the single summer (if any) who'll be no-offered. Keep plugging away for the final stretch, of course, but IMO you have no reason to be worried. Fingers crossed!

Again I think it depends largely on historical practice. There are some firms - even 100% offer firms - that don't give out offers on the last day, for whatever reason. I've even heard of firms that don't give out offers until a few weeks after.

Aside from bad reviews and no work (which from what I hear tend to be pretty blatant "you need to fix X or you are in danger of not getting an offer" talks, the only other bad sign I can think of, for firms where each group directly gives you an offer, is if you've done all your work for a group that turns out not to need an incoming associate.

And even so, I don't know that anybody has gotten a no-offer from that scenario. When I summered, there were some rumblings that we should be seeking work in groups that communicated an intent to hire first years (i.e. get corporate work, not trust and estates work), but even so, people ended up with offers in the group they wanted.

You should have been given an idea of the timeline for offers. My firm gives them a week after the summer ends.
