is it assumed that ALL cold-calling recruiters are to be avoided?

Title, basically.

Do legitimate/trustworthy/etc. headhunters ever do the cold-call/cold-email method?

I get from 2-4 a day or so, and certainly notice a disparity in the quality and how personalized these emails/call can be from different recruiters.

I've talked to a couple of them, nothing serious, but, not sure if I should bother with these cold-calling recruiters using an obvious spray-and-pray approach.

Or, is it just industry standard for even high quality headhunters to cold-call?

unless they are marketing something exclusive like wachtell or an in house role they are basically all the same and have the same access to all open positions in all major law firm. just make sure they don't apply to anywhere for you without your permission. very few really care about your career and will just try to place you as soon as you can so they collect their commission.

to answer your question, yes the recruiters for exclusive listings (e.g. like wachtell, inhouse, banking roles, etc) do spam as well. that's really the only way they can get enough qualified candidates to pass along. I've been spammed for each of the examples above.
