Overextending offers?

I assume firms overextend offers considering some offers will not be accepted. What happens if all of the offers are accepted though?

For large class-size firms, they simply absorb the year-to-year fluctuation.
Smaller firms/offices are often wary of overextending offers and deal with this by maintaining a waitlist so they never (at any given time) have more pending offers than they have spots for.
Bad firms will make a habit of overextending offers with a view to no-offering some fraction of the summer class. I understand this is more common with (although not exclusive to) lower/non V firms and certain legal markets (notably Texas).
