I had an IRS adjustment done over 7 years ago (but I can't recall the specific year) in which I had to pay the IRS more money. I am filling out my state bar application and I'm trying to answer it honestly. The issue is, I don't recall if I notified the state about the adjustment. I contacted the state and their records don't go that far back. Furthermore, the IRS has only provided me with the W-2s as filed with the IRS from those years and nothing indicating even what the amount of the adjustment was or whether or not I was provided an adjustment.

How do I disclose this information on a bar application that asks if I have filed all applicable income tax reports timely?
