Question about Job Application

Associate attorney here. Considering applying to a new governmental job. The job application requests my recent performance appraisal and contact information for supervisor. It says incomplete applications will not be considered. Although my appraisal was great, I did not receive any hard copy of it. I'm also hesitant to give my supervisor's information, as they might push me out the door for looking elsewhere.

How do I proceed?
