One of my interviewers is MD + JD...
do I call him/her Dr. Johnson? or Mr./Ms. Johnson?

Technically a 0L but I know a fair number of MDs. I'd say always better to go w/ Dr. If they correct you to Mr./Ms. it isn't a big deal, but any MD who reminds you s/he is a doctor is already annoyed.

Neither. Unless you know for sure otherwise, use first names if you're speaking with them in person. Lawyers will expect that and might think it's weird if you call them "Ms." or "Dr."

Agreeing with philosoraptor or disagreeing?

ALWAYS go with Dr. xxxx!!!! There's not even a question. Then if he/she asks you to call him/her by a first name or some other name, then go with that. You want to show respect for their medical degree by calling them "Dr.". I've worked with MDs for several years and regardless of whether or not they also have a JD and/or are practicing in another field (i.e., law), calling them "Mr./Ms." can be construed as insulting, depending on the person and their ego. Definitely be respectful and call him/her "Dr. xxx"

Edit: FYI, I'm a "Dr. xxx" myself I prefer when people call me by my first name, but it's always nice when someone knows my background and titles me accordingly. It shows they did their research and are conforming to professional etiquette. Obviously I immediately ask them to call me by my first name, but still... it gives a good impression of the person when they do that.

Go with Dr. Always err on the side of respect.
