I lost the NALP reimbursement form they gave me. I downloaded a new one but don't have the name/email address I'm supposed to send it to. Anyone have their copy?

Give them a call. If you're worried about this reflecting poorly on you, it won't and alternatively you can do it on a payphone or burner and give a fake name like Heisenberg.
