Cover letter when applying to multiple offices

Who do I address the cover letter too if I am applying to a multiple offices in a firm within a single firm...do I just put the firms headquarter office or just the name?

Depends on the firm. Generally it's inadvisable to apply to multiple offices of the same firm, because it makes you look indecisive. There can be exceptions (I know people who got offers from Skadden NYC and DC; some firms have multiple offices in small cities in one state that all make sense to apply to) but in general you'll have more leverage if your sales pitch is targeted towards Firm X in location Y as opposed to Firm X anywhere it happens to have hung a shingle, you know?

After that, it depends on the firm and the contact info you have for it. because of the above I would not assume you can just send one letter to a firm and assume it "counts" as applying to multiple offices; without more specific info I would just apply individually and directly to each office you are interested in, if you're sure you want to do that.
