Secondary Market Pay Scales
Do firms in cities that start at NYC market, like those in LA/Chicago/Boston, also pay NYC market to 2nd, 3rd, etc. year associates? Or do the salaries progress more slowly?

Vault has published guides to smaller markets which contain the sort of salary information you're looking for (Ex: Vault Guide To The Top Boston & Northeast Law Firms, Vault Guide To The Top Chicago & Midwest Law Firms, Vault Guide To The Top Southern California Law Firms). There are significant gaps, but a lot of the larger firms will be included, as will a few midsized firms. Some of the salary data might be out of date as the guides are only updated once every few years. Ask your career services office if they have copies/pdfs of these resources.
If you're only concerned about the top firms, ATL's career center directory have salary information for select firms in each market (ex: Ropes and Goodwin in Boston, K&E and Sidley in Chicago, Gibson Dunn and Quinn in LA). But they tend to only list salaries for the top firms which pay market. If you're interested in lower-end biglaw/midlaw firms, you'll have to do your research firm-by-firm.
