I know it varies among different judges. But general trend? And compared to fed district court?
Do they hire based on grades / writing samples / other factors?

Very selective. If you can land a FEDERAL CoA judicial internship I would take it and run.

I'm also curious. Some posters here told stories of people with CoA interns still ended up unemployed after OCI. Doesn't make sense if the criteria are similar for judges and biglaw i.e. grades/writing.

The general consensus is that 1L judicial internships are often not that selective and based on many random factors. The best legal job for 1L summer is whatever legal job you can get. Even better if it pays. The best indicator of success for 2L OCI is grades.
