C&F Issue - Academic warning/probation?

So I missed one deadline for my thesis draft during my graduate program a couple years ago. Nothing about it is mentioned on my official record but I did receive a warning letter by mail from my advisor stating that I'm lacking progress and falling behind the thesis requirements.

Is this considered as having been on academic probation or warning? Should I report this?

No, a note from your advisor saying that you're behind on an assignment (even a thesis) isn't covered by those questions. Academic warnings are official warnings from the university, not admonitions from individual teachers.

I would disclose everything. I seriously doubt they would reject your application for this.

Disclose everything that is responsive. The OP isn't describing something that is responsive to the question asked.
