Does each school require a Dean's Certification Form?

One of my four schools, Brooklyn, asked for a Dean's Certification Form bc i answered yes to one of their C an F questions in the application. So far this is the only school that has emailed me since I have submitted on 2/15/19 and I am currently in the process of getting them the proper forms, but will the other schools ask me for the same form. If so then I will be sure to tell my undergrad school to send out four instead of one. THANK YOU!

Your post is way too vague to give a definite answer. But it's extremely likely that Brooklyn (and the other schools you applied to) indicated what forms they needed to see in the relevant C&F portion of the application.
