You will need to disclose, and it may have an effect on your chances at admission. It will have a much greater effect if you lie about anything (including a lie by omission), and people have gotten in to T13 schools with worse infractions on their record.

cavalier's response is correct. You'll probably want to write a very brief addendum about lessons learned and point to your subsequent exemplary behavior as evidence.
