How to address academic probation

At one point in time, I was in a graduate program, but due to my own health reasons as well as needing to spend a significant amount of time as a caregiver, my GPA in the program dropped and I received an email from the program director stating that I would be placed on academic probation. After meeting with the director, I made the decision to withdraw from the program. When I looked at my online transcript, there is no mention of me being on academic probation, and honestly I'm not sure if I even was "officially" placed on academic probation, since I ended up withdrawing.
I've already written an addendum explain why my GPA was low, why I withdrew, and what I have been doing since withdrawing from the program (which is a lot).
My questions are:
1. In the C&F questions that talk about academic probation, should I say anything?
2. Will having to withdraw from this program have a huge negative impact on my application?
My uGPA is a 3.2 (science major) and LSAT is a 173.
EDIT: There's nothing on my academic summary stating that I was on academic probation. Should I just not mention it, and stick to the withdrawal?
