General Application questions

Hello yall!
I am ready to apply but i have some general/nitpicky questions! Much appreciated in advance!
1) High School GPA (Is this freakin required? I don't remember it at all gosh!)
2) Report all LSAT score right?
3) I worked for my friend's consulting firm for like 2 months (not a solid pay but would get 2-300$ as an appreciation). I put this on my resume but should i put this on my LSAC app?
